The Rapuora Mobile Nursing service will support whaiora māori to understand and manage their hauora (health). We will work alongside you if you have, or are at risk of getting heart disease, diabetes, or respiratory disease, whakapapa māori and are over 18 years old.
We will visit you at your whare and kōrero about your health needs. A health care plan will be developed together and will include information about your health, what you can do to support yourself and what other services you may benefit from. If needed we will work with other health services to help monitor your health.
Our nurses and support kaimahi will check in on a regular basis to support you monitor your health and make sure the plan is working for you.
In addition to in home and community care, Rapuora offers a Hauora programme to increase, physical, mental, social and cultural wellbeing of our whaiora. This includes low impact movement sessions, access to aqua exercise and exercise programs with our partners and weekly weaving sessions at our Eastgate site. These are free and we may be able to support with transport.
