The Rapuora Mobile Nursing service will support you to understand and manage your health. We can work with you if you already have, or if you are at risk of getting heart disease, diabetes, or respiratory disease, and you are over 18 years old.
We will visit you at home and discuss your health needs with you.
A health care plan will be developed with you. The plan will include information about your health, what you can do to support yourself and what other services you can access that may be helpful. We will liaise with other health professionals as required and help you to monitor your health. You can expect Rapuora to call on a regular basis to support you to monitor your health and make sure the plan is working out.
